 AccessHealth Spartanburg

South Carolina

 

AccessHealth Spartanburg is a multi-stakeholder, community-wide effort to address the unmet medical and social needs of uninsured individuals living in Spartanburg, South Carolina. AccessHealth care navigation teams screen potential clients for program eligibility (including eligibility for other local, state and national assistance programs); perform assessments for psychosocial needs; assist clients in creating lifestyle goals; collect medical histories; and assign medical homes tailored to clients' unique medical needs. This project is part of the Robert Wood Johnson Foundation's Transforming Complex Care initiative.
