Catholic Charities Health Guardians is an intensive and integrated care model that attempts to address the social, economic and health-related barriers that prevent high-need patients from receiving effective healthcare. The program uses intensive case management to integrate a range of services offered by Catholic Charities that include transportation, counseling, food assistance, transitional housing, referrals to behavioral health providers, workforce development referrals and health education. |

In a study done in conjunction with LSU HSC, since the program launched in 2012 in partnership with University Hospital (most recently called LSU Interim Public Hospital), over 72 percent of the clients in the study were able to decrease their ER and inpatient care visits by half. The estimated cost savings to the community is around $16,000 per client enrolled in the program. |
