Community Care Behavioral Health Organization

Community Care is a non-profit behavioral health managed care organization headquartered in Pittsburgh, Pennsylvania. Through a partnership with the Western Psychiatric Institute and other behavioral health providers, a team of mobile nurses and social workers coordinate primary care, specialty care and pharmacy services for the University of Pittsburgh Medical Center’s high-need patients. Additionally, care teams help patients develop health management strategies that include addiction recovery, tobacco cessation and weight management.
