Access Health at Sinai Hospital 

Maryland

 

Access Health is a partnership between HealthCare Access Maryland (HCAM) and Sinai Hospital that aims to address health disparities, reduce admissions, expand primary care and decrease the number of ER high-utilizers. Care coordinators embedded in Sinai Hospital's emergency department conduct detailed assessments of patients' social, economic and environmental needs and connect patients to the community resources necessary to address those needs. Additionally, care coordinators link patients with primary and behavioral healthcare providers, and provide follow up to maximize results. Care coordinators are supported by HCAM's internal case management system, that allows care plans to be shared with providers and insurance companies for ongoing care coordination.
