Homeless Patient-Aligned Care Teams (H-PACT) |

H-PACT clinics employ multidisciplinary care teams to provide Veterans with comprehensive, individualized care, including services that lead to permanent housing. Care teams are attuned to how housing insecurity and other social factors like poverty harm Veterans’ health overall, worsen sickness, delay care and exacerbate temporary and long-term homelessness. The H-PACT approach ensures that a fully integrated team is aware of and can treat the issues related and contributing to homelessness as part of the VA’s Housing First policy. |
