MaineCare Health Homes and Community Care Teams

The MaineCare Health Homes initiative began as an effort to deploy the medical home model to serve Maine's Medicaid patients. In 2013, the program began to incorporate Community Care Teams in order to target patients with multiple chronic conditions.  Based on the work of the Camden Coalition and prior demonstration projects, these interdisciplinary care teams provided high-touch, integrated care to targeted patients. In 2014, the program expanded to include Behavioral Health Organizations in order to treat patients with serious mental illness. There are currently ten Community Care Teams operating throughout the state.

Target Population

Medicaid enrollees or dual-eligibles with:

  • Two or more chronic conditions
  • One chronic condition (if they are at-risk for another)
  • Serious mental illness

Care Team

Case manager, nurse, physician, behavioral health specialist
