The Northern Colorado Health Alliance Regional Care Collaborative Organization employs a version of "hot spotting" to target high utilizing patients that would benefit from care management, including social services that are funded outside of Medicaid. Care managers become intimately involved in patients’ lives, accompanying them to appointments and meeting them in their homes. NCHA care managers focus on addressing barriers to care related to social determinants of health and ensure that members receive their basic needs as identified by Maslow’s Hierarchy. |
