Safety Net Medical Home Initiative

The Safety Net Medical Home Initiative was a national Patient-Centered Medical Home (PCMH) demonstration helping 65 primary care safety net sites across five states (Colorado, Idaho, Massachusetts, Oregon and Pennsylvania) become high-performing medical homes and improve quality, efficiency and patient experience. The Initiative created a framework for PCMH transformation and published a library of resources and tools to help practices implement the PCMH Model of Care.
