Transforming Complex Care Initiative

Transforming Complex Care is a 2-year program spanning six sites across the country that is funded by Robert Wood Johnson Foundation and led by the Center for Health Care Strategies. Transforming Complex Care aims to refine and spread effective care models that address the complex medical and social needs of high-need, high-cost patients. Participants will refine strategies across six key areas for improving care: care model enhancements; data and analytics; workforce development; financing and accountability; governance and operations; and policy and advocacy.
