University Hospital's Population Health Management Complex Case Program was launched in August of 2013. Using the Camden Coalition's hotspotting technique, staff identify ER super-utilizers based on medical history and frequency of visits, in addition to a willingness to make lifestyle changes. Once identified, a care manager provides eligible patients with a personalized care plan that connects them to needed medical and social services. |
