AC Care Connect

AC Care Connect is a pilot program (1115 waiver) that allows grantees to use Medi-Cal (California Medicaid) funding for non-standard services such as housing and "infrastructure" like information systems that support service delivery integration. The program is estimated to impact 20,000 individuals.

  • People experiencing homelessness
  • People who are high utilizers of multiple systems, including emergency services hospitalizations and law enforcement
  • People with complex conditions who need care coordination across multiple systems in order to obtain good treatment outcomes
