The Multicultural Health Foundation's Patient Health Improvement Initiative (PHII) began as a 3-year pilot program that uses a multidisciplinary team to improve health outcomes for complex patients. The care team works closely with patients for up to 1 year to stabilize persistent health conditions, address underlying social determinants of health and develop sustainable treatment plans to achieve the best long-term health outcomes possible. Doctors specialize in meeting the unique needs of African American, Latino, Asian and Middle Eastern patients. |
