The Multicultural Health Foundation's Patient Health Improvement Initiative

The Multicultural Health Foundation's Patient Health Improvement Initiative (PHII) began as a 3-year pilot program that uses a multidisciplinary team to improve health outcomes for complex patients. The care team works closely with patients for up to 1 year to stabilize persistent health conditions, address underlying social determinants of health and develop sustainable treatment plans to achieve the best long-term health outcomes possible. Doctors specialize in meeting the unique needs of African American, Latino, Asian and Middle Eastern patients.

Target Population

Chronically ill people living mainly in the medically underserved communities of Southeast San Diego, City Heights and National City who are high utilizers of emergency rooms and are frequently hospitalized
