Sutter Health's ED Enhanced Case Management Program |

The ED Enhanced Case Management Program encompasses a number of initiatives aimed at reducing unnecessary hospital admissions. The program includes a Care Transitions nursing program as well as the nationally-recognized Sutter Care Coordination Program that has operated since 2006. In 2016, Sutter Health and Alameda Health partnered to launch the PreManage ED program, which provides super-utilizers with a portable health record and care plan. The system alerts six area hospitals whenever a frequent utilizer is admitted at any one location. |
