Verde Valley Caregivers provides volunteers, programs, and services to support adults in need of assistance in maintaining their quality of life at home. Their Transitional Care Program provides intensive support to high-cost, high-need patients via a nurse-led transition team that provides specialized support services to patients 30-60 days after discharge from Verde Valley Medical Center, including in-home visits and numerous phone calls. The team provides "intensive community social services" such as housing referral and served 670 patients from Oct. 2012 - Dec 2015. |
