Metro Community Provider Network's Bridges to Care Program |

Metro Community Provider Network is a Federally Qualified Health Center providing primary care and other services to the medically underserved and uninsured populations in the suburbs surrounding Denver. Using the Camden Coalition's “hot spot” model, care teams visit patients in high-need, high-cost neighborhoods to improve their access to healthcare and help manage their chronic conditions (see: Camden Coalition, Multistate). The network provides general primary care, physical examinations, child visits, immunizations, chronic disease management, family planning, limited pharmacy and laboratory support and referral to outside services. |

Among some 225 B2C program graduates, the number of ED visits to UCH decreased 49 percent, while the number of hospitalizations fell 55 percent, resulting in savings of nearly $8 million. |
