Northland Healthcare Alliance's Care Coordination for Seniors

Northland Healthcare Alliance's Care Coordination for Seniors program aims to improve patient care, at-home independence and quality of life for seniors with home-health needs. Patients are assigned a care coordinator who conducts home visits and can help with medication adherence, securing home safety devices and communicating with the patient's care team. Non-medical in-home care includes light housekeeping, transportation, personal care and connection to assistance programs such as meal delivery. The program has served over 875 patients.

Medicare beneficiaries or Medicaid enrollees ages 55 and older

Care coordinator, in addition to nurses, therapists, social workers and other healthcare professionals as needed
