Foundational Community Supports is a 5-year Medicaid demonstration project that helps Medicaid enrollees with complex health needs find supportive housing and supported employment. Payments do not include room and board, but pay for services that help enrollees obtain and maintain housing, including one-time supports for individuals to avoid institutional settings and transition into an apartment or home. The initiative's success will be measured based on its ability to improve health measures, improve employment statistics, provide more stable living situations, reduce use of intensive care services, produce cost savings and improve the overall health of communities. |
