Foundational Community Supports

Foundational Community Supports is a 5-year Medicaid demonstration project that helps Medicaid enrollees with complex health needs find supportive housing and supported employment. Payments do not include room and board, but pay for services that help enrollees obtain and maintain housing, including one-time supports for individuals to avoid institutional settings and transition into an apartment or home. The initiative's success will be measured based on its ability to improve health measures, improve employment statistics, provide more stable living situations, reduce use of intensive care services, produce cost savings and improve the overall health of communities.

Target Population

  • Supportive housing services: Medicaid enrollees who complete a needs assessment and: have experienced chronic homelessness, depend on costly institutional care, depend on restrictive adult residential care/treatment settings or are in-home care recipients with complex needs
  • Supported employment services: Medicaid enrollees that have physical, behavioral or long-term service needs that make it difficult to obtain and maintain a job
