Genesys HealthWorks’ “Familiar Faces” program serves high-risk, frequent ED users by supporting patient self-management, particularly in developing health behavior changes. This program evolved out of an innovative "health navigator" program that Genesys developed through several pilots starting as early as 1997. The program initially targeted patients with diabetes and asthma, but expanded to include include smokers, patients with chronic diseases and those with acute needs upon enrollment and post-emergency department visits. |
