Healthy Alliance Independence Practice Association

MVP Health Care (a managed-care organization) and Alliance for Better Health (a convener of providers and community groups) have partnered to invest $800,000 over two years in not-for-profit community organizations around Albany, N.Y. that provide services to address social determinants of health. Provider and community-based organization partners will use a shared technology platform to send and receive referrals, enabling the association to track who is using social services and determine whether the services have led to improvements in health.

  • 29 community-based organizations, including a food pantry, supportive housing center and homeless shelter

Medicaid beneficiaries and others living in Albany, N.Y. with unmet social needs
