The Lehigh Valley Super Utilizer Partnership (LVSUP) provides intensive outreach and care coordination for patients with complex illness who have been admitted to the hospital two or more times in the past 6 months. The program aims to help patients navigate complexities in the healthcare system, build relationships with their providers and accomplish goals that will ultimately improve their quality of life. This pilot is based on the Camden Coalition's model. | |
