New York's Medicaid Health Homes program is a care management model that provides a variety of services to enrollees with chronic health problems through a network of providers, health plans and community-based organizations. With the help of a central case manager, the program aims to reduce trips to the emergency room and time spent in the hospital, encourage regular care from medical providers and help people find stable housing and transportation. Eligible Medicaid beneficiaries are identified through analysis of claims and encounter data. In 2016, the model was expanded to serve children. |
