Maximizing Independence (MIND) at Home

Multistate

Maximizing Independence (MIND) at Home is a comprehensive, home-based care coordination intervention for people with Alzheimers disease or related dementias who live in the community. The program links people with dementia and their caregivers to community-based agencies, medical and mental healthcare providers and other community resources. Borne out of series of research projects, the program's most recent demonstration projects are a federally-funded trial of an intensive version of MIND at Home called MIND-Plus and a privately-funded randomized controlled trial to evaluate a leaner, more targeted version of MIND at Home called MIND-Streamlined.
