One Care is a demonstration project jointly funded by Massachusetts' Medicaid program and the federal Centers for Medicare and Medicaid Services that targets dual-eligibles under age 65 that have serious mental illness, substance abuse issues or disabilities. The program seeks to integrate medical, behavioral and social services for these patients using care coordinators to connect patients to care provided at the physician's office, at home and in community settings when possible. Care coordinators work with each patient's providers to create a Personal Care Plan to address physical health, mental health, substance abuse and long-term services and support needs. The program has operated at a loss in its first two years but anticipates savings in 2017. |
