Show Me Healthy Housing

Missouri

 

Missouri Foundation for Health's Show Me Healthy Housing Initiative provides funding to four health-related nonprofit agencies to construct affordable housing communities for vulnerable Missourians with complex health needs and their families. In addition to healthcare and case management services provided on-site, residents are connected to a variety of services to improve behavioral and social determinants of health. These include mental health services, treatment for substance abuse disorders, permanent rental subsidies, employment assistance and parenting/life skills coaching. As of 2018, the foundation has funded the development of four supportive housing communities (in Columbia, Hannibal, Springfield and Mexico) that have begun accepting tenants.

Target Population

Vulnerable individuals with complex health conditions who are: Veterans, seniors, disabled and/or low-income. Individuals and families may have histories of homelessness or Serious Mental Illness
