The South Central PA High-Utilizer Learning Collaborative is a collaborative of five regional health systems that are implementing programs (guided by the Camden Coalition's model) that aim to provide super-utilizers with integrative medical, behavioral and social services. The partnership involves the Crozer-Keystone Health System, Lancaster General Health, Pinnacle Health System, Wellspan Health and Neighborhood Health Centers of the Lehigh Valley and Widener University. Each hospital has its own method of implementation: Lancaster General Health's "Care Connections", Crozer-Keystone Heath System's fellowship program, PinnacleHealth Systems "Community Health Navigation Network", Neighborhood Health Centers of the Lehigh Valley's "Lehigh Valley Super Utilizer Partnership," and WellSpan Health's "Bridges to Health." |
